Gastrointestinal metastasis oflhe lung cancer is an unusual finding but is found in 1-4% of the patients at the autopsy. Recently, several cases who were diagnosed as gastric metastasis during life were reported. We experienced a case of Pancoast type lung cancer (adenocarcinoma) and multiple adenocarcinomatous lesion in the gastrointestinal tract. Patient was a 39-year-old man who presented with left upper chest and arm pain. Chest x-ray showed a mass in the left apex. Aspiration cytology revealed a poorly differentiated adenocarcinoma. During a course of radiotherapy, he developed severe anemia. Gastroscopy revealed multiple fungating masses with umbilication in the esophagus, stomach and duodenum.
